Do I need Centennial?

Centennial Pharmacy Services is a medication management-based pharmacy with a team of clinical pharmacists and pharmacy technicians who are trained to support your needs. You can benefit from Centennial’s support if you:

  • Have multiple chronic conditions (like diabetes and heart disease).
  • Take multiple prescribed medications.
  • Get overwhelmed with the organization of your medicines, or have made mistakes like taking the wrong medicine at the wrong time or not taking your medicine at all.
  • Have trouble getting to your pharmacy to pick up your medicines.
  • Rely on family, friends, or paid caregivers to help with your health care.
  • Are currently in a transition from a hospital or facility or are frequently visiting the ER or hospital.
  • Are restricted in your activities of daily living such as bathing, dressing, eating, cooking, managing your finances, cleaning, doing laundry, or using transportation.
What is a CarePack?

A CarePack is a date and time stamped pouch that has all of your medicines in it for a specific time of day. For instance, all of the medicine that you take in the morning will be packaged together in a pouch labeled for morning.

What does Centennial cost?

Centennial’s pharmacy services and delivery do not have any additional costs. The only payment ever collected from Centennial is your insurance co-payment, co-insurance, or deductible.

Your chosen insurance plan determines your benefits and copayments. Centennial Pharmacy Services accepts most major insurance plans. If Centennial is not currently in your insurer’s network or is not a preferred pharmacy for your plan, you can still use Centennial’s services and request that the cost of your medications be provided from Centennial in writing for a reimbursement request from your plan. You will always be notified of your prescription copays prior to being billed.

If you request vitamins or other over-the-counter items not covered by your insurance, you will be charged the amount for that item.

  • Check: Via mail or at time of delivery

What if my medicines change?
Centennial will contact your doctor(s), verify your new prescription regimen, and make the necessary changes to your CarePacks.
